29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:     Jobsite located at 10356 S 124th Street, Papillion, NE 68046: The employer is failing to ensure that employees engaged in residential construction activities were protected from falls while working at heights greater than 6 feet from lower levels. On or about June 9, 2015; employees of Jose Angel Jimenez dba G&J Roofing were observed performing residential roofing activities without conventional fall protection while exposed to fall distances greater than 16.3 feet on the upper section of the roof and 11.4 feet next to the ladder.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Jobsite located at 10356 S 124th Street, Papillion, NE 68046:  The employer is failing to provide a training program to ensure that employees are able to recognize and minimize hazards when exposed to falls greater than 6 feet. On or about June 19, 2015  employees of Jose Angel Jimenez dba G&J Roofing were observed performing residential roofing activities without conventional fall protection while exposed to fall distances greater than 16.3 feet on the upper section of the roof and 11.4 feet next to the ladder without protective measures such as but not limited to guardrails, safety nets and or personal fall protection systems.

29 CFR 1926.1053(b)(1):  Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder(s) was/were not secured at its top to a rigid support, and/or a grasping device, such as a grab rail, was not provided to assist the employees in mounting and dismounting the ladder:    On the jobsite located at 10356 S 124th Street, Papillion, NE 68046: The employer is failing to ensure that a Keller extension ladder used by employees extended at least 3 feet past the roof level being accessed and/or is failing to ensure the ladder was secured from accidental displacement.

29 CFR 1926.1060(a):  The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these:    Jose Angel Jimenez dba G&J Roofing specifically on the jobsite located at 10356 S 124th Street, Papillion, NE 68046:    The employer is failing to provide employees with training relating to the safe usage of ladders, the hazards associated with unsafe usage and the procedures to follow to minimize the hazards. On June 19, 2015 employees were observed utilizing a Keller extension ladder to access a roof to perform work. The ladder was not secured to prevent displacement and the ladder did not extend at least 3 feet past the roof being accessed
