29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:    a) East end of building, on or about 6-11-15:  Two employees were installing roof trusses while standing on the top plate of the exterior wall partitions, exposing the employees to a fall hazard of 10 feet 6 inches and 9 feet 10 inches.     b) East end of building, on or about 6-11-15:  Two employees were installing roof trusses while standing on the bottom chords of the trusses, exposing the employees to a fall hazard of 9 feet 10 inches.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) East end of building, on or about 6-11-15:  Two employees were installing roof trusses while standing on the top plate of the exterior wall partitions, exposing the employees to a fall hazard of 10 feet 6 inches and 9 feet 10 inches.     b) East end of building, on or about 6-11-15:  Two employees were installing roof trusses while standing on the bottom chords of the trusses, exposing the employees to a fall hazard of 9 feet 10 inches.
