An employee working from a pump jack scaffold working platform was exposed to a fall hazard of 20-feet 3-inches as the support poles of the pump jack scaffold system were bearing directly on the ground and not on the base plates or any other adequate firm foundation.

An employee working from a pump jack scaffold working platform was exposed to a fall hazard of 20-feet 3-inches as no guard rail or safety net system had been installed to protect the open ends and edges of the working platform. Additionally, the exposed employee was not wearing a personal fall arrest system.
