29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   (a) Southeast side of building 200 located at 400 Belmont Place SE, Smyrna, Georgia. An employee using a pneumatic nail gun, without eye protection, while installing a metal flashing drip edge, was exposed to eye injuries from flying debris.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   (a) Southeast side of building 200 located at 400 Belmont Place SE, Smyrna, Georgia. An employee working from an elevated aerial lift to access was exposed to a fall hazard of approximately 21-feet 5-inches. The employee was not wearing a body belt or full body harness, which should have been attached, by a lanyard, to either the basket or boom of the lift.
