29 CFR 1910.334(a)(2)(i): Portable cord and plug connected electric equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):     (a) On or about 6/23/2015, at the facility, for the director of housekeeping that use the spray buff machine, the electrical cords that power the machine had insulation damage.     (b) On or about 6/23/2015, at the facility, for the physical therapists that use the hydrocollator (hot pack machine), the electrical cord that powers the hot pack machine had insulation damage.     (c) On or about 6/23/2015, at the facility, for the aid that uses the fan near the sink, the electrical cord that powers the fan had insulation damage.
