29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a)  At 8001 Berry Road in Bonne Terre, MO 63628 - On or about 06/09/2015, employees working in areas where there was a possible danger of head injury from falling shingles and roof material being thrown from the roof above, were not protected by protective helmets (hard hats).     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a)  At 8001 Berry Road in Bonne Terre, MO 63628  On or about 06/09/2015, each employee working on a steep roof (5 in 12 pitch or greater) with unprotected sides and edges, approximately 11 feet above a solid concrete surface, was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems.      NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident:    a)  At 8001 Berry Road in Bonne Terre, MO 63628 - On or about 06/09/2015, an employee who was fatally injured during steep-slope commercial roofing, was pronounced dead at 3:45 P.M. on 06/09/2015, and the employer did not report the work-related incident within 8-hours.
