29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.    (a) Roof of the house at Lot # 5, 148 Blakmore Drive, Smyrna, GA: Employees were exposed to fall hazards while improperly using a harness and carabineers as a positioning belt with a rope grab located at the wearer's waist.  The employees were on a 8:12 pitched roof at a height of approximately 24 feet from the eave to the lower level.

29 CFR 1926.1053(b)(1):  A portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) West side wall of the house at Lot # 5, 148 Blakmore Drive, Smyrna, GA: The employer was using an extension ladder to access the roof of a single family house that was not extended three (3) feet above the landing exposing the employees to fall hazards.
