29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: a) Roof, Residence, 9 Lafayette Street, Avis, PA - Employees engaged in repairing and shingling the low sloped roof of a residence roof were exposed to a fall hazard that ranged from ten-foot (10') to fifteen-foot, four-inches (15'4") above ground level as observed, on or about June 15, 2015. In accordance with 29 CFR 1903.19(c), abatement certification is not required for this violation. The hazardous condition was corrected during the inspection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) North side, Residence, 9 Lafayette Street, Avis, PA - Employees used a twenty-foot (20') aluminum extension ladder that extended two-foot (2') above the roof eave to access the residence roof, as observed on or about June 15, 2014. In accordance with 29 CFR 1903.19(c), abatement certification is not required for this violation. The hazardous condition was corrected during the inspection.
