29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about June 12, 2015, at or near 211 Hood Street, West Monroe, LA; the main switch used to isolate a machine from the electricity supply was broken. The switch was turned on and off using a pair of pliers. This use was not consistent with the listing and/or labeling of the electrical equipment.
