29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.  On or about 15 June 2015, and at times prior thereto, an electrical extension cord was being used to provide power for hand-held tools in which the cord had visible strain relief damage at the plug end, exposing workers to an electrical hazard of approximately 120 volts.
