29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not used:  On or about June 15, 2015, employees were installing the siding with air nailers at approximately 18 feet high on a pump jack scaffold without fall protection, hard hats or safety glasses.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level by fall protection established in accordance with paragraphs (g)(1)(i) through (vii) of this section:  On or about June 15, 2015, employees were installing the siding on the apartment buildings under construction approximately 18 feet high on a pump jack scaffold without fall protection, exposing the employee to a fall hazard.
