On or about June 16, 2015, at the above addressed jobsite, a powered industrial truck (Hyster #5) had a latch for the propane tank missing, exposing the employees to a fire hazard. 2. On or about June 16, 2015, at the above addressed jobsite, a powered industrial truck(Hyster #2) had an inoperable back-up alarm, exposing employees to a struck-by hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about June 16, 2015, at the above addressed jobsite carpentry department, an extension cord lacked strain relief where its cord entered into the attachment plug, exposing employees to an electrical hazard.
