29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:       Jobsite located at 9359 Dewey Avenue, Omaha, NE 68114: The employer is failing to ensure that employees engaged in residential construction activities were protected from falls while working at heights greater than 6 feet from lower levels. On or about June 16, 2015; employees of MB7 Construction, L.L.C. were observed performing residential roofing activities without conventional fall protection while exposed to fall distances greater than 16.5 feet on the upper section of the roof and 7.7 feet next to the ladder.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     On the jobsite located at 9359 Dewey Avenue, Omaha, NE 68114: The employer is failing to ensure that a Werner aluminum extension ladder used by employees extended at least 3 feet past the roof level being accessed and/or is failing to ensure the ladder was secured from accidental displacement.

Jobsite located at 9359 Dewey Avenue, Omaha, NE 68114: The employer is failing to provide a written fall protection training certification that included the signatures of the person(s) who conducted the training or the signature of the employer; for each of the employees working on the roof. On or about June 16, 2015; employees of MB7 Construction, L.L.C. were observed performing residential roofing activities without conventional fall protection while exposed to fall distances greater than 16.5 feet on the upper section of the roof and 7.7 feet next to the ladder.
