29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:     a. On or about June 17, 2015, at 3179 W. McPherson Highway; Fremont, Ohio:  In the warehouse, adjacent to the staircase leading to the supervisors office, a junction box was missing a cover plate, wires were sticking out of the junction box exposing employees to an electrical hazards.    b. On or about June 17, 2015 at 3179 W. McPherson Highway; Fremont, Ohio:  In the Warehouse, near entrance door, a four duplex outlet cover plate was  was not used or installed in accordance with manufacturer's instructions. The cover plate was  not secured to the outlet box leaving an opening in the wall plate there by exposing employees to an electrical and fire hazards.
