29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):     Employees performing operations, housekeeping and maintenance tasks were exposed to amputation hazards in that the drive pulleys were not guarded for the following locations:  a)    On the mixing/main floor of the mill the drive pulleys on the west side of the crumbler machine were not guarded. b)    On the mixing/main floor of the mill the drive pulleys on the east side of the crumbler machine were not guarded. c)    On the mixing/main floor of the mill the drive pulleys for the feed cleaner were not guarded. d)    On the mixing/main floor of the mill the drive pulley for the roller mill was not guarded. e)    On the shaker floor the three sets of pulleys mounted on the one drive shaft for the cleaner were not guarded. f)    In the tunnel under the concrete grain bins the end pulley for the open conveyor belt was not guarded.

29 CFR 1910.272(j)(2)(ii): The employer did not immediately remove any fugitive grain dust accumulations whenever the exceeded 1/8 inch (.32cm) at priority housekeeping areas, pursuant to the housekeeping program for grain elevators:  Employees working in the grain elevator and feed mill were exposed to fire and explosion hazards in that dust accumulations were in excess of 1/8 inch:  a)    In the basement of the headhouse on an old motor mount pad approximately 14 feet away from the main bucket elevator legs, dust and grain measured 8.5 inches deep. b)    In the basement of the headhouse on the floor approximately 9 feet away fro the main bucket elevator legs, dust measured 8.5 inches deep. c)    In the basement of the feed mill approximately 1 foot away form the pellet bucket elevator leg, dust measured over 2 feet deep. d)    In the headhouse on the top floor 3 feet away from the main bucket elevator legs, dust measured 3 inches deep.
