29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146:    A)    Siferd Plumbing Heating & AC Services Co., LLC, conducting work at the private residence located at 6210 Township Road 254, Arcadia, OH 44804: On or about June 16, 2015, the employer did not develop and implement a written permit-required confined space program. Employee were required to enter the wastewater treatment systems tank in order to conduct repairs and were exposed to the hazards such as, but not limited to, atmospheric hazards (hydrogen sulfide and carbon monoxide). A written permit-required confined space program shall include elements such as, but not limited to:  ª Measures to prevent unauthorized entry;  ª Hazard identification and evaluation;  ª Means, procedures, and practices for safe permit entry operations including the specification of acceptable entry conditions, provisions for authorized entrants or their authorized representative to observe any monitoring or testing, isolation of the space, purging, inerting, flushing, or ventilating as necessary to eliminate or control atmospheric hazards, and verifying that conditions in the permit space are acceptable for entry throughout the duration of an authorized entry;  ª Necessary equipment;  ª Evaluation of permit space conditions;  ª Providing attendant(s);  ª Designation of active roles (authorized entrants, attendants, entry supervisors) in entry operations;  ª Procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and for preventing unauthorized personnel from attempting a rescue.
