29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:   a) At the jobsite: 1300 South Street - The employer used a Pettibone 1157 Extendo Rough Terrain Forklift to elevate an employee within a manmade wooden box approximately forty (40) feet from the ground, exposing the employee to fall hazards. The wooden box was not approved by the manufacturer. Observed on or about 05/08/2015.

Observed on or about 05/08/2015.
