29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:           a) Parker Ave Bridge, Passaic, NJ: Two employees were exposed to an engulfment and/or entrapment hazard while in a culvert with water approximately 12 feet below the road surface and conducting a bridge inspection without wearing personal flotation device (PFD). On 06/15/2015, an employee drowned while conducting a bridge inspection.
