29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by the following machines:    a. Mixer Line #1. The mixer had interlocked grates that were bypassed. This allowed the employees to gain access to the mixing blades while the machine was in operation. This exposed employees to falling into the rotating mixing blades and being caught in.     b. Mixer Line #2. The grates did not have guards to prevent employees from gaining access to the rotating mixing blades when the machine was in operation. This exposed employees to falling into the rotating mixing blades and being caught in.     c. Mixer Line #3. The grates did not have guards to prevent employees from gaining access to the rotating mixing blades when the machine was in operation. This exposed employees to falling into the rotating mixing blades and being caught in.     d. Bottle Hopper Line #1. The Bottle hopper was not guarded to prevent employees from being caught in and crushed by the hopper when it is lowered.     e. Barry Wehmiller. The machine was not properly guarded to prevent employees from gaining access to the point of operation. This exposed employees to a caught in and pinch point hazard.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
