29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: (A) A specific statement of the intended use of the procedure; (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: The employer's documented procedure for employees assigned to perform servicing and maintenance on industrial drying ovens was not adequate while servicing and maintenance was conducted by more than one employee isolating multiple energy sources. Employees were exposed to burns from hazardous energy sources. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: Employees were assigned to perform meat cutting operations on bandsaws with guards that did not prevent contact with non-working (unused) portion the moving blade. Employees were exposed to inadvertent contact with the blade while making repetitive cuts as a continuous part of the production process and were likely to sustain amputation of their fingers and/or hands. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures. Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations shall be installed so as to prevent moisture or water from entering and accumulating within the enclosures and shall be mounted so there is at least 6.35-mm (0.25-in.) airspace between the enclosure and the wall or other supporting surface. However, nonmetallic enclosures may be installed without the airspace on a concrete, masonry, tile, or similar surface. The enclosures shall be weatherproof in wet locations. In wet locations subject to regular sanitation including deluge with water and cleaning agents, the employer failed to ensure that electrical boxes and fittings were installed so as to prevent moisture or water from entering and accumulating within. Enclosures were not weatherproof. Employees were exposed to electrical shock hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws: Hanging pendant cords were not provided with devices or fittings that relieved strain from being transmitted to the wire terminals within the receplacle ends of flexible cords.
