29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  Instance 1: On or about June 17, 2015, at the Home2 Suites by Hilton Parc Lafayette construction site on Kaliste Saloom Road in Lafayette, LA, the employer did not ensure that the plug on the power cord of the Milwaukee Hole Hawg (Serial No. 413ED14261980) which employees used to drill holes in wood was equipped with a ground pin.
