29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.  Location - 206 Ridge Street, Winchester, MA:  On or about 6/19/15, the employer did not perform a frequent and regular inspection of the job site to protect workers from the following but not limited to hazards;  1. Fall hazards of up to 20-feet, 3-inches while working from the roof without fall protection; 2. Fall hazards of up to 20-feet while working from the roof with inadequate fall protection; 3. Fall hazards of up to 12-feet, 3-inches while workers performing work from a low sloped roof without fall protection; 4. Fall hazards of up to 12-feet, 11-inches while workers utilizing three portable extension ladders that were damaged, un-even and un-secured, set-up at an improper angle, and 1 ladder that did not extend 3-feet above the upper landing. 5. Housekeeping hazards of debris, trash, protruding nails that had not been removed from the work area; 6. Overhead struck by hazards from workers performing work on the roof and workers below not wearing hard hats; 7. Home-made scaffold not constructed and erected by a designated qualified person; 8. Eye injury hazards from workers operating pneumatic nail guns and a circular saw without safety glasses.
