The chemicals labeled corrosive to the eye include, but is not limited to, VORACOR CM 759 Additive. This condition exposed employees to chemical hazards.     Instance A: In the chemical storage house, where the isocyanate and polyol is unloaded, the eye-wash station was located outside the room on the right side of the door. In the event of an emergency employees would open a door that is normally kept closed to get to the eye-wash station.     Instance B: In the foam house, where employees pour the chemicals to the panels, a suitable eye-wash station was not available. In the event of an emergency, employees would have to walk approximately 87 steps to the nearest eye-wash station that allowed for 15 minutes of continuous flush.
