29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  At the job site 8887 Green Valley Drive, Manhattan Kansas, the employer had not developed a written lockout tagout program to protect employees from the hazards of stored energy or unexpected start up of equipment such as: Slotter cutting machines, CNC cutting machines, and the compressor.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by unguarded saws and rotating cutting machines.  At the job site 8887 Green Valley Drive, Manhattan Kansas, employees were operating the equipment listed below without proper guarding.   Employees were exposed to amputation hazards.  1. All 5 CNC machines in the production area were being operated without the door interlocks functioning. 2. Slotter 4A-4B was not fully guarded at the cutting area. 3. Slotter #3, Double Head Cutters were not fully guarded at the cutting area. 4. Slotter #2 cutting area was not fully guared. 5. Enclosed Slotter #1 had opening at the end that was not closed.

You must also consider a case to meet the general recording criteria if it involves a significant injury or illness diagnosed by a physician or other licensed health care professional, even if it does not result in death, days away from work, restricted work or job transfer, medical treatment beyond first aid, or loss of consciousness.  At the job site 8887 Green Valley Drive, Manhattan Kansas, the employer had not keep on file the OSHA 300 Logs and OSHA 300 A summaries for 2015, 2014, and 2013.
