29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:  a)  Fleet Maintenance Shop, on or about 6/22/15:  An industrial floor fan did not have a permanent and continuous path to ground and was being used at the time of the inspection for general cooling purposes. The metal casing of the fan had voltage present when tested with a voltage detector.  b)  Battery Charging Area, on or about 6/22/15:  The battery of an electric pallet jack was observed being charged. The battery plug was connected to an extension cord that had a missing ground pin.  The path to ground from the pallet jack was not permanent, continuous and effective.  The metal frame of the pallet jack had voltage present when tested with a voltage detector.  c)  Battery Charging Area, on or about 6/22/15:  An extension cord used daily to charge an electric pallet jack had a missing ground pin from the plug end.  The path to ground from pallet jacks was not permanent, continuous and effective when connected to this extension cord and charging.
