29 CFR 1926.95(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:   On or about June 23, 2015, an employee was exposed to the hazard of being struck by flying projectiles, in that he was using a pneumatic stapler to attach building wrap to the building without the use of safety glasses.

29 CFR 1926.100(a):  Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about June 23, 2015, an employee was exposed to the hazard of being struck by falling objects, in that the employee was working below residential construction activities, and was not wearing a hard hat to protect against falling material and debris.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:  On or about June 23, 2015, employees were exposed to a fall hazard of approximately 20 feet to the ground level below, while attaching building wrap to a residential construction from a JLG telescopic boom aerial lift without a personal fall arrest system being attached to the basket.
