29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  On June 23, 2015, employees were not protected from falling through skylights while performing roofing operations at a commercial building, exposing employees to a fall hazard of 19.5 feet and the employer did not ensure employees were adequately protected from a fall.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  On June 23, 2015, employees were not protected from a fall while performing roofing operations at a commercial building with a flat roof, exposing employees to a fall hazard of 19.7 feet and the employer did not ensure adequate protection was utilized.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On June 23, 2015, an employee performing roofing operations was exposed to fall hazards up to 19.7 feet and the employee had not been trained to recognize or minimize the hazards associated with a fall.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
