On June 23, 2015, employees were not protected from falling through skylights while performing roofing operations at a commercial building, exposing employees to a fall hazard of 19.5 feet and the employer did not ensure employees were adequately protected from a fall. On June 23, 2015, employees were not protected from a fall while performing roofing operations at a commercial building with a flat roof, exposing employees to a fall hazard of 19.7 feet and the employer did not ensure adequate protection was utilized. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
