29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a)  Jobsite at 13 Chester Street, Glens Falls, NY - On or about June 24, 2015 employees were on a steep roof and exposed to a fall hazard of approximately 38 feet and not protected by either a guardrail system, safety net system or personal fall arrest system.

29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:  a)  Jobiste - 13 Chester Street, Glens Falls, NY 12801 - On or about June 26, 2015 workers were replacing slate shingles on a steep slate roof exposed to a fall hazard without the benefit of inspection of the jobsite by a competent person designated by this employer.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)  Jobsite at 13 Chester Street, Glen Falls, NY 12801 - On or about June 26, 2015 the employer had employees exposed to a fall hazard on a slate roof without providing them with a training program concerning the falls hazards and procedures to avoid falling.
