Verona Dr., Covington, LA.; On or about June 24, 2015, the employer allowed an employee to operate a Makita hand grinder to cut roof tiles while not wearing safety glasses or a face shield for protection from the flying debris.

29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels, and paragraph (d) of this section.  a) 485 N. Verona Dr., Covington, LA; On or about June 24, 2015, the employer allowed an employee to roof tiles using a Makita hand grinder that was missing the safety guard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 485 N. Verona Dr., Covington, LA; On or about June 24, 2015, the employer allowed employees to work on a roof top while not wearing fall protection or having the edges protected by a guardrail system. Employees were installing tiles on the roof.
