29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) On or about 06/16/2015 in the shape mill; where an employee was seriously injured during the cleaning of the mill rolls, on the large shape mill, while the mill rolls were turning. The employer did not ensure that equipment was shut down, deenergized and locked out before cleaning the rolls and the employee's hand and arm were pulled into the in-running nip-point causing serious injuries.  ABATEMENT DOCUMENTATION REQUIRED
