29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered.  At 425 East Whitestone Boulevard, Cedar Park, TX, 78613, on or about 15 June 2015, two employees were performing power washing activities at the edge of a low slope, metal roof without fall protection, exposing the workers to the hazard of falling approximately 18 feet to the ground below.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  At 425 East Whitestone Boulevard, Cedar Park, TX, 78613, on or about 15 June 2015, two employees were performing power washing activities without using eye protection exposing them to the hazard of being struck by flying objects.
