29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  At the inspection site, where employee(s) and/ or contractors were using hazardous chemicals including, but not limited to, Brazilian Blowout Acai Professional Smoothing Solution containing methylene glycol, the employer did not develop and implement a written hazard communication program, exposing employees to the inhalation and contact hazards of chemicals, including formaldehyde, an IARC Group I carcinogen.

29 CFR 1910.1048(n)(3)(i): The training program for all employees exposed to formaldehyde did not include a discussion of the contents of this formaldehyde regulation and/or the contents of the Material Safety Data Sheet:  At the inspection site, where there was potential for exposure to formaldehyde at or above 0.1 parts per million (ppm) during Brazilian Blowout treatment, the training program did not include a discussion of the contents of this formaldehyde regulation and the contents of the Material Safety Data Sheet for Brazilian Blowout Acai Professional Smoothing Solution, exposing employees to the contact and inhalation hazards of formaldehyde, an IARC Group I carcinogen.  Note: A short-term exposure limit (STEL) of 0.1497 ppm formaldehyde was measured during the blow drying phase of a Brazilian Blowout treatment on July 7, 2015.
