29 CFR 1926.451(f)(15): The employer did not ensure that ladders shall not be used on scaffolds to increase the working level height of employees, except on large area scaffolds where employers have satisfied the criteria listed in (15)(i) through (15)(iv): a) Employees were exposed to ladder/scaffold collapse hazard when standing on a 6 ft stepladder placed on top of a 6 ft tall baker scaffold to paint the 14 ft high ceiling. The ladder and scaffold tipped over. Location: 683 Dean Street, Brooklyn, N.Y.
