29 CFR 1926.651(b)(1): The estimated location of underground utility installations, such as sewer, telephone, fuel, electric, water lines, or any other underground installations that reasonably may be expected to be encountered during excavation work, were not determined prior to opening the excavation:  a) 1935 - 1939 West George Street, Philadelphia, PA 19130: The employer failed to determine the location of underground installations prior to opening the excavation. Employees struck and damaged a buried sewer line while digging. Conditions occurred on or about June 23, 2015.

29 CFR 1926.651(b)(2): Utility companies or owners were not contacted within established or customary local response times, advised of the proposed work, and asked to establish the location of the utility underground installations prior to the start of actual excavation:  a) 1935 - 1939 West George Street, Philadelphia, PA 19130: The employer failed to call utility companies to identify the location of underground installations prior to opening the excavation. Employees struck and damaged a buried sewer line while digging. Conditions occurred on or about June 23, 2015.
