29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not seperated from fuel gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire resistant rating of at least one-half hour:   A.  On or about July 6, 2015, New London Store #179, Shop:  In the following locations, oxygen and acetylene cylinders were not separated by a minimum distance of 20 feet or by a 1/2 hour rated fire wall five feet high.  1.  Bay 1, on a welding cart.  2.  Against the north shop wall.

On or about July 6, 2015, New London Store #179, rear wall of shop:  1.  Three 15A 125V relocatable power taps were used against listing and labeling instructions in that they were powered through a flexible extension cord and plugged into each other.  2.  Kenmore 2.4 cubic foot refrigerator was used against listing and labeling instructions in that it was powered through a relocatable power tap.  Listing and labeling instructions require the equipment to be plugged directly into an outlet.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  A.  On or about July 6, 2015, New London Store #178:  1.  Back Room:  The General Electric electrical panel was missing a breaker in position #12, leaving an uncovered opening.  The door on the panel was not locked and employees were exposed to the energized 120V bus bar.  2.  Bay 2:  A four-receptacle outlet box mounted on a building support in Bay 2 was missing a knockout on the top, exposing employees to the energized inner conductors.

On or about July 6, 2015, New London Store #178, Tire and Storage Room:  The side-hinged door at the back of the building was not provided with a sign indicating it was not intended to be used as an emergency exit.
