29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about June 06, 2015 at the residential construction site located at 22615 Timber Ridge Drive, Lot 300, Livingston, Louisiana, the employer allowed employee(s) to conduct roofing activities on a 10 in 12 pitch roof without fall protection. The measured distance from the eve of the roof to the ground was nine feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 06, 2015, at the residential jobsite located at 22615 Timber Ridge Drive, Lot 300, Livingston, Louisiana the employer allowed employee(s) to access the residential roof at a height of nine feet without the ladder extending at least three feet above the upper access level. The employee(s) were conducting roofing activities.      The Blanco Roofing, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.1053 (b)(1) which was contained in OSHA inspection number 313030934, citation number 1, item number 1a and was affirmed as a final order on 8/25/2010, with respect to a workplace located at LA 621 and Lakeway Dr. intersection, Gonzales, LA. 70737.
