29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) On or about 25 June 2015 at FP Management/Sterling Apartments at 4018 Harvard Ln, Kansas City MO 64133: Employer failed to develop and implement a lock out/ Tag out program. An employee was exposed to electric shock hazards and electrocution.            Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings must be provided with covers identified for the purpose. If metal covers are used, they must be grounded. In completed installations, each outlet box will have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or will have smooth, well-rounded surfaces on which the cords may bear.    (a). FP Management 4018 Harvard Lane Kansas City MO, second floor power closet.  The employer failed to cover all outlet boxes exposing employees to shock and electrocution hazards.       Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
