The injury was not recorded on the employer's OSHA 300 log.    c.) Facility - On or about 6/20/15, an employee suffered a low thoracic strain, which resulted in 1 lost day.  The injury was not recorded on the log.    d.) Facility - On or about 4/15/15, a TEMPORARY employee was splashed in the R eye with chemicals which resulted in irritation/chemical burn and subsequent medical treatment.  The injury was not recorded on the OSHA 300 log.    ABATEMENT CERTIFICATION REQUIRED
