Periodic inspections were not performed for any equipment within the facility in which employees would be required to lock out and tag out. Hazardous energy sources include but are not limited to electrical, mechanical, hydraulic, and pneumatic. Equipment within the facility includes but is not limited to, envelope presses, slitters, and balers.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): On or about September 30, 2015, the employer failed to ensure that the electrical, pneumatic, and mechanical energy disconnects for the envelope lines were operated in a manner that isolated the energy sources prior to authorized employees performing work, in that locks were not applied to each specific energy source nor were the energy source(s) relieved, disconnected, restrained, or otherwise rendered safer during servicing/ maintenance and set up operations. Numerous envelope lines were observed during the inspection being serviced and maintained by employees without such actions being performed.
