29 CFR 1910.178(l)(1)(i): The employer had not ensured that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by completion of the training and evaluation specified in paragraph (l) [Reference 29 CFR 1926.602(d) for construction]:  a.  On or about June 30, 2015 on the southeast side of a new construction commercial building, the employer did not ensure that an employee was trained and evaluated in the operation in forklift operations, exposing employees to struck-by, crushing hazards, and fall hazards.
