29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:       a) 114 Edgegrove Avenue, Staten Island, NY:  Employees were observed utilizing pneumatic powered tools, including but not limited to a Hitachi nailers, during roofing operations without the use of eye or face protection.     Condition observed on/or about June 30, 2015.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:       a)114 Edgegrove Avenue, Staten Island, NY: Employees were observed performing roofing operations at a height of approximately 30' to the ground below with no fall protection in place, exposing them to fall hazards.      Condition observed on or about June 30, 2015.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:    a) 114 Edgegrove Avenue, Staten Island, NY: Employees were observed utilizing a 40' Werner aluminum portable ladder to access the roof that had bent/broken rungs, which could effect the structural stability of the ladder, exposing them to fall hazards.      Condition observed on or about June 30, 2015.
