29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more:    a. Site - employees were not protected from falls of approximately 17 feet when employees were required to climb onto a 25 inch parapet in order to get to and from the ladder which was used for access to the roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a. Site - Employees were not protected from falls of approximately 17 feet when an aluminum extension ladder used to gain access to and from the roof was only extended 6 inches above the landing surface.
