OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to the hazard of falling while using a fiberglass extension ladder:     WORKSITE: The employer did not ensure through providing instruction, training and visual observation that the Louisville Model L3121-20, Type 1 20-foot fiberglass extension ladder that was used by an employee to access the roof was set up properly, such as (but not limited to) placing on firm foundation/surface, positively securing, and extending the side rails at least 3 feet above the landing surface to prevent from accidental displacement while being used.    On or about May 7, 2015, the above ladder was displaced while an employees was climbing up the ladder to access the roof, and the employee fell 12-1/2 feet to the ground and sustained multiple injuries from the incident.
