29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  On or about June 24, 2015 the employer did not ensure that two employees were required to use fall protection when working from an aerial lift platform. Employees working above 10 feet from ground level were exposed to fall hazards.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On or about June 24, 2015 the employer did not ensure that two employees were trained on the safe operation and hazards associated when working off an aerial lift. Employees working above 6 feet from ground level were exposed, but not limited to fall hazards.
