29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  Location:  9 Andrews Road, Greenwich, CT 06830    Each employee exposed to fall hazards of approximately 23 feet to the ground while working (painting a dormer while on the roof) on a 10 in 12 pitch roof was not provided with training in the hazards associated with falls and the procedures/means to be followed in order to minimize and/or eliminate such hazards.

Location: 9 Andrews Road, Greenwich, CT 06830    The Louisville 40 foot aluminum extension ladder set up at a 44 degree angle in order to access and exit the roof of the house was not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder.
