29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On or about June 10, 2015; southeast corner; employees framing a new residential home were observed walking the outer edge/top plate without fall protection in place to prevent them from falling an estimated 30 feet to ground level. b) On or about June 10, 2015; second floor; employees were working near the window openings which were not properly guarded exposing him to an estimated 25 foot fall to ground level. The Dunnings Carpentry dba Danny's Carpentry was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13), which was contained in OSHA inspection number 1050915, citation number 1, item number 1 and was affirmed as a final order on (date), with respect to a workplace located at Macomb Street NW Washington DC, 20007.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) On or about June 10, 2015; throughout the site, employees were operating electrical equipment such as but not limited to portable circular saws that were plugged into a portable extension cord that the strain relief was damaged exposing the insulated wires.
