29 CFR 1926.501(b)(3):  Each employee in a hoist area was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems or personal fall arrest systems:      a)  Employees were exposed to the hazard of falling approximately 12.6 feet to the ground while in the process of hoisting an HVAC unit onto the roof.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   a)  Employees were exposed to fall hazards while hoisting an HVAC unit and the employer did not provide a training program to recognize fall hazards and the procedures to be followed in order to minimize those hazards.
