29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    175 Main St, Agawam MA  On July 10, 2015 and at times prior, employees operated pie press machines that were incompletely guarded allowing access to the point of operation form under and over guards.  On June 11, 2015, an employee reaching under a pie press guard to straighten a piece of plastic needed for product quality, received a fingertip amputation.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.    175 Main St, Agawam MA    As of July 2, 2015 at the start of the inspection, the employer had not reported to OSHA that on June 11, 2015 an employee operating a machine received a fingertip amputation.
