Quick Stop Fuel Station - On or about July 2, 2015, personal fall protection equipment was not worn by employees removing components from the fuel station sign while working from a JLG Telescopic Boom Lift (model: 400S), thus exposing employees to a fall hazard of up to 15.25 feet onto earthen ground.
