29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  a)  On 06/21/15, in the Secondary Department, an employee was exposed to amputation hazards, when the Reed Thread Rolling machine was not shut down ("locked-out"), when a thread roll failed to open.  This employee was not trained as an "Authorized" employee when unjamming this machine.  ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.147(c)(7)(iii)(A): Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the energy control procedures.  a) On 06/21/15, in the Secondary Department, an employee was exposed to amputation hazards, when the Reed Thread Rolling machine was not shut down ("locked-out"), when a thread roll failed to open.   Employees Affected and Authorized were not provided with "Lock-Out" procedure retraining on the Reed Thread Rolling Machine, after machine modifications.   ABATEMENT CERTIFICATION REQUIRED
